1. Purpose

To keep tailings storage facilities, dams, ponds, pipelines and water systems stable and compliant, and to catch early signs of failure or release.

2. Scope

Tailings storage facilities, process water ponds, sumps, pipelines and discharge points.

6. Procedure

Routine inspection

  1. Walk the wall, crest, toe and spillways on the set schedule. Record water levels and freeboard.
  2. Look for cracks, slumping, wet spots, seepage, erosion, animal burrows and vegetation changes.
  3. Check pumps, pipelines, valves and flow meters.

Monitoring

  1. Read piezometers, inclinometers, survey points and flow meters and record them.
  2. Compare with trigger levels and report trend changes the same day.

Response

  1. On an alert trigger, increase inspections and tell the Geotechnical Engineer.
  2. On an action trigger, restrict access, lower the pond level and start the emergency plan.
  3. Report to the regulator as required by law and the permit.

7. Stop-work triggers

Stop the job, make the area safe and call the supervisor if any of these happen:

  • Freeboard falls below the minimum
  • New cracks, slumping or muddy seepage appear
  • Monitoring reaches an action trigger
  • A pipeline leaks or a spill occurs

8. Emergency response

  1. Raise the alarm and tell the Incident Controller and Responsible person.
  2. Evacuate people in the inundation zone and stop the discharge.
  3. Start the emergency action plan and notify authorities and downstream communities as planned.
